Icon-only treatment has been user tested countless times by thousands of organizations in many locales and proven to be effective.

In usability testing, the metric is not 'was user was momentarily confused' but rather 'can user complete task'. I invite you to place labels on all icons in the examples provided and user test yourself.

You will find that rather than being more intuitive, the additional visual clutter of 10pt text reading 'BACK' 'DOWNLOAD', 'DELETE', 'MAIL', 'MORE' will make the interface less clean and less usable.

No, we don't need to label the hamburger icon 'MENU' because it adds little/no value to usability. In the case where concept does not have an intuitive icon, place under a 'MORE' menu and present in a drop-down list.
